As part of a settlement, Sound Construction agreed to submit monthly lists of active worksites and allow random OSHA inspections.

However, during the June 12 follow-up inspection in Stamford, OSHA found that the company’s reckless behavior persisted. Inspectors identified seven willful and four serious violations related to excavation hazards. Shockingly, the company still hadn’t trained workers on unsafe trenching practices, provided adequate cave-in protection, or required daily excavation inspections. Additionally, they failed to follow proper trench shield installation standards and didn’t backfill shields to prevent hazardous movement.

These latest violations resulted in proposed penalties of $1,224,798—more than triple the amount from the fatal incident investigation.
